Calendar Form Instructions

If you plan to email your Calendar Order Form to us, PLEASE READ THIS FIRST. Here are the steps:

  1. Make sure you have installed Adobe Reader (it's free from adobe.com)
  2. Install CutePDF Writer (this is freeware) at cutepdf.com
    • Go to cutepdf.com and click on Free Download (under PDF Creation)
    • You'll be asked whether to Run or Save (Choose Save)
    • Save to Desktop
    • Install CutePDF Writer
    • You don't have to Install the Ask Toolbar (just uncheck, keep moving, and click next)
    • Click Install (you will probably have to restart your system after installation)
  3. Now, instead of printing to your favorite printer, you can print to CutePDF Writer (which will save all the information in your order form as a PDF document - just make sure you name it filename.pdf - not literally, but make sure it has the .pdf extension)

If you're ready to start the process, here are the steps:

  1. Download Order Form, click here (if you're filling out the form within your browser, go to step 5)
  2. Save to Desktop
  3. Launch Adobe Reader (if you don't have it, it's free - to download & install click here)
  4. Open Order Form (File/Open 2012CalendarFormRev1.pdf)
  5. Fill in Your Information (using the tab key to move to the next field)
  6. Use arrow keys to select Method of Payment (then tab to the next field)
  7. When Finished, you can Print (you cannot save it) the Form on Paper or to a File; using CutePDF Writer (to email as an attachment, follow instructions below)

To Save Your Completed Form to a PDF:

  1. Follow the instructions above (you must do this FIRST or else, it won't work)
  2. While you have your completed form on screen, File / Print
  3. Instead of your favorite printer, select CutePDF Writer in the Name field and click OK
  4. Make sure you name the new file something like, FirstInitialLastName.pdf (i.e. cbrown.pdf)
  5. Save in My Documents (so you'll know where to find it when you attach it to your email to us)
  6. Voila! You should be good to go!
  7. If this seems too hard, simply print out your completed form, then call, fax or include payment and U.S. Mail it to us
